Registration Agreement (High School)

I understand and agree to the following:

1. The Head Coaches must adhere to CIF policies on coaching during "off-season" play.

2. An Ace 5on5 Roster/Liability Form is required to be signed by each players legal parent/guardian.

3. Players may not participate without parent/guardian authorization.

4. Ace 5on5 reserves the right to validate signatures on all Roster/Liability Forms.

I agree to these policies and I agree to inform
all of my parents/guardians of their signature obligation.
